I confirm I am over 18 years of age and I am the owner or co-owner of the above named dog and I am entitled to make this agreement.

Please read through the following terms and conditions. Any owner who does not agree to these terms – I’m afraid Dogaholics will be unable to accept their dogs as guests. This document forms a contract between Dogaholics and the dog owner and is binding on both parties.

All guests are required to have:

• Up-to-date vaccination cards
• Up-to-date kennel cough vaccination cards
• Up-to-date with their worm treatments
• Up-to-date with their fleas treatment

Please bring your vaccination cards with you on your first visit.

Clean and Fresh

Because we are a home environment:

• All four-legged guests must be: CLEAN, WASHED, GROOMED and SMELLING FRESH please.
• All bedding must be: CLEAN, WASHED and SMELLING FRESH please.
• If both of the above could take place ONE DAY prior to your dog/s stay we would be very grateful.

 

Kennel Cough Unfortunately, Kennel Cough - which is a highly contagious disease that spreads rapidly - is on the increase.

The disease is not only spread by contact with infected dogs, it is also spread by airborne transmission - so the name is misleading, as it is not always caught in kennels. The disease is so contagious that dogs can also catch it whilst out on walks with just brief contact with an infected dog.

Kennel Cough must be administrated TWO WEEKS prior to your dogs stay.

Fleas and Worms

All dogs must be treated by veterinary approved Flea and Worm treatments before their stay. All treatments must be administrated at least ONE WEEK prior to dog's arrival.

In the event of a Flea or Worm infestation throughout your dog’s stay we will treat our guests at the owner’s expense with one of the following veterinary approved treatments.

• Frontline
• Stronghold
• Advocate
• Advantix
• Other – please state

Please state which WORM treatment your dog is currently on

Veterinary Information and Details Should any problems arise throughout your dog's stay your vet will be notified and your pet will be transported to their surgery for treatment.

A Vet-to-Pet transportation fee will be charged at £25.00 per hour plus 40p per mile.     

In the unlikely event of an emergency throughout your dogs stay, and your vet is either too far away or unable to see your dog - your dog will be taken to:

In the event of this happening the owner will be liable for any veterinary fees incurred.

 In the unlikely event of an accident or surgery DOGAHOICS will immediately notify you on any emergency telephones numbers provided advising you of the attending veterinary surgeon comments.

Should we be unable to contact you on the emergency telephone numbers provided - agreement is permitted for DOGAHOLICS to make any decision necessary regarding the dogs health provided it is acting in the best interests of the dog/s and is the advice of the qualified veterinary surgeon.

• A FULL day's fee will be charged on and including your arrival and departures dates unless your dog departs before 10am.
• A 25% deposit is required on ALL bookings at the time of confirmation.

• Your booking will NOT be secured until a cheque has been received.
• Please make cheques payable to 'Sammie Luck'.

• The balance on all bookings must be paid in FULL on the FIRST DAY of your dog’s arrival.
• Payments are to be made by cash or cheques only. 

• Any uests staying on a BANK HOLIDAY, CHRISTMAS EVE, CHRISTMAS DAY, BOXING DAY, NEW YEAR'S EVE and NEW YEAR'S DAY will be charged at an additional rate.

• Should you decide to collect your dog before the agreed collection date, you will be charged for the full term booked.

Cancellations

• Cancellations made less than 21 days prior to ALL booking start dates, will result in full loss of the 25% deposit.

• Cancellations made less than 14 days prior to ALL booking start dates, will result in the FULL amount being charged.  
• Cancellations must be made in writing by post or via e-mail. A message left on an answering machine or via text message, will not be accepted as a cancellation.

 

Health and Safety

Please note: Dogaholics main concerns are for the welfare, happiness, safely and comfort of our guests at ALL times as well as client satisfaction and endeavour to give you and your dog the best service we can possible give.

 DOGAHOLICS DISCLAIMER:  

All reasonable skill and care will be taken when attending to your dog. All advice is given without prejudice and your dog exercised under constant supervision.

I HEREBY CONFIRM THAT I AM THE OWNER OF THE ABOVE NAMED DOG(S) AND I HAVE AUTHORITY AND CAPACITY TO MAKE THIS AGREEMENT AND THAT I AUTHORISE DOGAHOLICS TO ACT AS GUARDIAN DURING MY ABSENCE – ALLOWING DOGAHOLICS - TO TAKE ANY ACTIONS NEEDED WHICH THAT THEY CONDSIDER SUITABLE IN ORDER TO PROTECT AND KEEP IN GOOD HEALTH THE ABOVE DOG/S AND I DO FURTHER CONFIRM THAT I WILL BE RESPOSIBLE FOR ANY COSTS INCURRED, EITHER VETERINARY OR OTHER, AS A RESULT OF ANY SICKNESS, ACCIDENTAL DAMAGE CAUSED TO OR BY THE ABOVED NAMED DOG/S. I ALSO ACCEPT THIRD PARTY LIABILITY AND I WILL PAY ANY SUCH COSTS DAMAGES OR EXPENSES ON DEMAND.

THE EXPRESSION ‘OWNER’ INCLUDES A CO-OWNER OR CO-OWNERS AND THIS AGREEMENT TAKES EFFECT AS IF BOTH OWNERS AND ALL OWNERS AS THE CASE MAY BE HAVE SIGNED WITH SIGNATURE BINDING ON ALL OWNERS WHETHER SINGLY, JOINTLY OR OTHERWISE UNDER THIS AGREEMENT.

DOGAHOLICS ACCEPT NO LIABILITY FOR ANY LOSS DAMAGE DEATH OR INJURIES HOWSOEVER CAUSED INCLUDING BY NEGLIGENCE OR OTHERWISE. THIS CLAUSE COVERS THIS AGREEMENT AND ALL OTHER  ARRANGEMENTS AND AGREEMENTS FOR SERVICES UNDER THIS AGREEMENT OR ANCILLIARY TO IT INCLUDING DOG WALKING ON AND OFF THE LEAD AND ALL SERVICES OFFERED ACCEPTED AND DELIVERED BY DOGAHOLICS  INCLUDING BY THIRD PARTIES AS MAY ARISE.

NO LIABILITY IS CREATED UNDER THE CONTRACT (RIGHTS OF THIRD PARTIES) ACT 1999.

THIS AGREEMENT IS TO BE CONSTRUED ACCORDING TO THE LAWS OF ENGLAND.

 ARRIVALS and DEPARTURES

Please conform to the agreed arrival and departure times of your booking as other dogs and guests need to be considered.
